Over time, active facial muscles may result in harsh lines on the face. Some of the lines for men and women include scowl lines, frown lines, and crow’s feet. These facial lines will deepen and get worse with aging. But with treatments that effect the muscle strength, fine lines will soften.

These medicines work by weakening the muscles. There are two types of lines on the face. There are dynamic lines and there are static lines. A dynamic line is the first type of line to appear on the face. It occurs with a motion of the facial muscles. The line goes away once the motion stops. An example is a smile line that occurs with a smile that goes away after the smile is gone. A static line means that the line is there even if the face is not moving. These are the lines of age. They take time to develop. A static line occurs after years of motion. So a line down the side of face that started by smiling but remains when not smiling is a static line. These treatments are designed to stop a dynamic line from happening. If they stop them, then there will not be a static line.

Botulinum toxin type A (BOTOX© Cosmetic by Allergan Inc.) is a purified and sterilized extract from bacteria. It is FDA approved to treat the area between the aging eyes in a specific muscle area. It can also be used ‘off-label’ in other areas. Plastic surgeons and opthamologists have used it for years. It is used to treat various medical conditions, especially around the eyes. It is used to weaken overactive facial muscles. It is best to treat facial lines before the lines become too deep.

BOTOX© Cosmetic works by not allowing facial muscles to contract. This means that the facial muscles are not able to cause the dynamic facial lines. Again, if there is no dynamic line, then a static line can not appear. BOTOX© Cosmetic is injected into the area of the muscles. The effects will occur in two to five days. Over the following days, the facial lines will soften. These lines can go away with repeated treatments. The timing is based on the severity of the facial lines. Results can be dramatic and usually last 3 to 6 months. The length of time that the injections last also depends on other factors. The patient’s genetics, amount injected freshness of the product, and the skill of the injector.

BOTOX© Cosmetic treatments done in proper conditions will last longer each session because the muscles will atrophy over time. So a later treatment of the same dose will result in longer lasting. This is true as long as the treatment occurs before complete facial muscle activity returns. If the facial muscle activity returns completely, there will be no increase of the interval between treatments. There is no limit to the number of treatments that you may have. If you decide to discontinue them, you will have a return of your muscle activity. Side effects of a BOTOX© Cosmetic treatment may include:

  • temporary facial bruising
  • tenderness at the injection site
  • headache
  • eyelid droop
  • brow asymmetry
  • forehead wrinkle asymmetry
  • other facial asymmetry

These types of issues are also temporary and can usually be adjusted by a change the amount of product used during the next treatment.


Dysport is also a sterilized extract from bacteria that has FDA approval. Dysport is made from Botulinum toxin type A. Similar to BOTOX© Cosmetic, it can be used to treat facial scowl lines, facial frown lines, and crow’s feet. It basically has the same use profile as BOTOX. It works the same way and there is little difference between the products. Some patients like one product more than the other. We use both products often with good results. Developed in the early 1990’s, Dysport was first used in Europe and is still being used under the name Reloxin. Over the years, it has proven to be safe and effective. Dysport is a competitor to BOTOX© Cosmetic and is essentially used the same way. It is more concentrated than BOTOX© Cosmetic.
